Noble B's answer is WRONG. Although it is true that mini-DVI doesn't output audio, either mini-TOSlink or USB *will*. So you need a mini-DVI & USB or mini-DVI&mini-TOSLINK to HDMI adaptor.
The mini-displayport&USB to HDMI are much easier to find, but keep googling and you'll find the mini-DVI versions. (easier to find the mini_TOSlink than USB)
So if you have a receiver that lacks assignable inputs, there are indeed solutions for getting audio and video onto the HDMI cable... just not easy to find ones.- Answered by Geo S from La Jolla

You don't. The A1181 (the older Macbook) only had Mini DVI, not Mini Displayport, and as such, does not support audio through the display connector. You can use a standard 3.5mm to stereo RCA cable or an optical TOSLink to minijack cable if optical is an option.
