Using this case, can you plug in accessory units such as a card reader or ear piece into the top?

Yes! All ports are open with nothing impeding them. The bottom and top have complete cutouts for access to inputs, and the volume buttons are in a cutout area as well. But since the sides of the case stick up just a hair from the front of phone, it still does a good job protecting it.
