Is there a difference in the interface between using a TC or an Airport Extreme with an external HD?

Or does the external HD attached to an Airport Extreme show up in the same way as a Time Capsule's HD?

The time capsule offers better security, automatic individual user accounts with both "shared" space and private space. It also is a server grade hard drive that operates much faster and is more reliable than a standard USB hard drive. So, in essence: While they are both methods of providing wireless storage, the time capsule is going to give you a much faster and more reliable experience. You have to remember that running a server hard disk that is accessed by more than one computer at a time is much more demanding than a simple disk connected to just your computer.
