How is this mounted on a desk? Is it necessary to drill holes on the desk?

The stand utilizes a mounting plate which requires five holes be drilled through the desktop. Four small holes at 9/32" and one large 5/8" hole.

Yes it is necessary to drill the desk. The holes can be repaired with liquid wood filler or similar and the surface could ultimately be restored. a total of 5 holes must be drilled with 4 smaller 9/32" and 1 larger of 5/8". Long and short, consider what you are drilling into.
