I can confirm the answer is yes. Took me less than 10 mins to install it in my MacPro1,1 (original Mac Pro).

Yes it does, it comes with two power cables to connect it from the card to the logic board.

It comes with two auxiliary power cables with 6-pin connectors to connect with the two auxiliary power connectors in the PCI area.
