can the universal dock adapter be use while the iPhone is in a Otter case

The short answer is no.
The universal dock adapters are made to fit their intended devices and are molded to fit the actual device. They cannot begin to make dock adapters to fit the multitude of sleeves and cases that exist. Some cases are made where the end close to the dock connector can slide off. The Otter Box cases like the Impact, Commuter or Defender cases are meant to be installed and left on. Repeated removal and installation can eventually tear or weaken the case.
