What is the difference between Apple TV and connecting my iPad to a TV via the lightning to hdmi cable for watching on demand movie services?

Cost, convenience, and reliability. Apple TV starts at $99 while the Lightning to Digital AV (HDMI) adapter only costs $50. An Apple TV will be more convenient in the long run, unless you don't mind having to connect your iPad each time and walk up to it to change anything. As far as reliability, I've seen mixed reviews for the adapter. It works for most people, but some people can't get it to work or have poor resolution (mostly on older devices).
Basically the adapter is cheaper as long as it works, but an Apple TV might be easier to use and has more functionality.
