The answer depends on the model of your MacBook and the TV.
This will work for most recent MacBooks that have the 3.5mm mini optical port. It puts the optical fiber at the tip of the normal headphone jack (3.5 mm mini) and allow the same port to be used for both digital optical and analog headphones. This cable ships with a TOSLink to 3.5 mm adapter according to the description. So it should work with your MacBook.

No, it doesn't work.
I have the Bowers & Wilkens speakers that I tried to connect to my A/V receiver via its Optical Out port, and using the RCA adapter on the speaker-end of the Toslink cable (input into the AUX port on the speaker). No sound at all, not even feedback. :-(

Your MacBook's external video out connection does not transfer any audio with the video signal (ie it's just video). To transmit audio as well, you'll need to establish either an analog audio link to your TV, or a digital link using this cable. The latter will only with if your TV has a digital optical in port.
For analog audio, you would need a cable with a standard 1/8" (3.5mm) headphone TRS connector for the Mac end (this is sometimes called a "stereo mini-jack"), and dual RCA phono plugs (red & white) for the analog audio in ports on your TV.
