The "SmartFold" SCR3500 mini USB reader is a fully ISO7816 compliant contact smart card reader. Due to its compact design the SCR3500 is ideally suited as a mobile reader device that can easily fit in the user's pocket or be attached to a key ring.

This is just the hardware device (smart card reader) like you would purchase other readers to access your CAC on any platform. This device allows the physical card to be accessed by software running on the computer via the USB port. This reader is CCID compliant and can be used on any platform.
Any software (Native in the OS -OR- from third-party providers) would be required to use the certificates & private keys on the card to perform tasks such as signing forms as you asked. No. This device is for a "Smart Card" similar in form factor to your Bank Card or Credit Card, but with a hardware "chip" exposed on one side. Smart Cards are used for various User Authentication scenarios like login on a computer, digitally signing/decrypting email messages and accessing PK-enabled websites.
