best app to assemble and present photos in pdf format

I want to use my new IPad in the field to do building inspections. I have to take several photos then mount the photos and make a brief description of each. This needs to be then sent to client in PDF format. I also have to fill out a form in PDF and upload that as well. What would be the best app for this?
