If you have to use the auxiliary connection for audio, how exactly is this device bluetooth? Either it is wired or it's bluetooth, not both.

Answer
It is a wired connection from this device to your car's auxiliary input jack and Bluetooth (wireless) between your phone or iPad to the Bluetooth adapter. This way you can listen to music without your phone or iPad having to have a wired connection between itself and you car's auxiliary input jack.
