*The following terms and conditions govern this offer: End-user customers must order and take possession of qualifying Apple products from March 28, 2004, through June 26, 2004. Apple products must be purchased directly from Apple or an authorized Apple reseller located in the 50 United States or the District of Columbia.

- This offer is not valid with the purchase of used or refurbished equipment.
- You must be an end-user purchaser, and not a reseller, to obtain this promotional offer.
- Customers must purchase BOTH a qualifying Power Mac G5 computer and a 23-inch Apple Cinema HD Display AT THE SAME TIME ON THE SAME SALES ORDER to be eligible for the mail-in rebate.
- To receive a rebate you must provide: (1) A completed request coupon. (Request coupons may not be altered.) (2) A proof of purchase consisting of itemized, dated sales receipt(s) or invoice(s) showing BOTH Apple products purchased, the reseller’s name and address, your name and address, the serial numbers of the qualifying products, and the price paid for the qualifying products. NOTE: Order acknowledgements,packing slips,and purchase order copies do not qualify. Handwritten invoices or sales history receipts will not be accepted as proof of purchase. (3) The original UPC/bar code labels from all Apple qualifying product boxes.The UPC/bar code labels must be cut from the product packaging so that the cardboard where the labels are attached is completely removed from the packaging. The UPC/bar code labels must include part numbers with bar codes, serial numbers with bar codes, and product descriptions.
- If you are submitting multiple promotions, the original cutout UPC/bar code labels must be sent with the first claim. Please note on subsequent claims the name of the promotion for which the cutout UPC/bar code labels were mailed.
- Lost, late, or misdirected mail is not the responsibility of Apple Computer, Inc. or its agents.
